We have 2 children of school age, and once a week, they have a private music lesson, at home, with a teacher. Shall we report her and pay national insurance contributions on her behalf?

A private lesson, held either at the home of the student or the teacher, is not included in the definition of domestic work and, therefore, you don't have any report and payment obligation. This is also true for those who conduct workshops and classes and those who teach in any field, including Torah lessons and Bar Mitzvah preparation.
